CSS Form Builder server problems...

Ok! So, I downloaded my extensions ok and everything seemed well until I decided to use CSS Form Builder and for some reason I keep getting the error message in one of the images attached herewith.

The first image shows the result I get when I test the preview but the second image is the result I get when I test the web page locally with my testing server.

It looks as though the input tags are being broken out of for some reason. The extra code you see if for the client side validations that occur. If you can please post back with this form page in a zip archive so that we can take a look and try to discover what the cause is.
