setup for MySQL php connection

This may be very basic but i'm having difficulty getting past stage one of setting up a database connection. I have added a mysql database on my server and put in the required details via the connection wizard but i get an error message when i test, thus preventing me to go any further. Any help would be appreciated. I couldn't find any documentation on this sections..... i presume its basic.
