Javascript errors when trying to insert a CSS formbuilder form

I am using Dreamweaver CS4 on Mac OSX. When trying to insert a CSS form I am getting Javascript errors and nothing will happen. I have applied the patches as defined in your sticky post. Help and a solution much appreciated as I am trying to finish a site for a client.
