WAFV_Required_C.htm not present

I'm still looking into the javascript error I get when I try to apply client side "Required" validation. One obvious problem is I do not have the WAFV_Required_C.htm file present anywhere on my PC. I do not see where I can acquire this file so I am hoping you can post it here.

Scratch that request. I found it in C:\Documents and Settings\[user name]\Application Data\Adobe\Dreamweaver CS4\en_US\Configuration\Commands

But I still have the javascript error. :(

On the Download page for CSS Form Builder , look in the known issues section. There is a zip file there with an updated version of the WAFV_Required_C.htm and instructions on where to place it to fix the error.
