CSS Form Builder (Simple)

Please review the attached picture of the inserted Simple Contact Form through using the Form Builder.

1) I am having problems centering the form.
2) When I filed out the form including satisfying the security related captacha and questions. I clicked the submit button -- it did not do anything it just stayed there, both the captcha and the security question, then change to new ones. This happened several times. If you are wondering, yes, I filled it correctly. I must have done this at least six times.

Q: Are there other things that I need to configure for the form to be abble to submit?

**** I have since found a way to make it work.

My next problem is: How do I set the form up to make it re-direct to a "thank you" page after a submission is made? The way it is now, the contents of the form is displayed on the screen when a submission is made.

You will need to use Universal email to send the information that was entered in the form as an email. in universal Email, there is an option to tell it which page to redirect to after the email is sent.

Hi Jason -- I have done as you suggested. I did apply Universal Email. The form simply will not submit. It kept saying the the answers to the security were not matching. I know that this is not true because I did it several times.

What I think is happening is that: the form is getting confused when the submit botton is pressed -- hence the message about mismatch texts.

It could be due to several things -- one of which could be that my settings were wrong on the Universal Email set up.

On the form itself the ACTION was set to "form092609.php" -- this was what it pointed to when the form was generated by the FB.

Please find below the picture of the form over the internet. Also let me know if you will need other files from me.

Here is the run down. I received a call from Jimmy -- Both he and I could not understand why the form was failing. However Jimmy suggested to build a new form, native to FB -- without applying any template.

These are the thing that we observed.

FB crashed during the first attempt to build a fresh contact form. This led me to close DW CS4 -- then deleted the WinFilecache. DW CS4 was reloaded and another attempt to build a fresh contact form. This attempt succeeded and the Universal Email was applied. When form was loaded onto the server. The form was tested -- we found that, not all of the form loaded. -- It was missing all the form section. So we went back to the code and discovered that a lot of codes were missing. The missing codes were manually added. Form was uploaded to the server -- tested form, and it worked.

The big question is why did we need to make so much code "add" to the form to make it work. Jimmy will be following up with this.

Later on after the conference meeting with Jimmy. I closed out DW CS4. Deleted WinFileCache. I then went on to delete the FB extension from the Ext Manager. Finally I reloaded a fresh FB DW Extension.

Relaunched DW CS4 -- I then built a new Contact form. It worked with out any problem this time around.
