Captcha not stopping SPAM form submissions

I have recently added Captcha to a clients contact form. It is not stopping SPAM/BOT submissions. I believe we had used a second verification (2 step) security question but I don't remember how we accomplished this. Is this the best path to take and if so, is there documentation on "how to" implement this 2nd step. Thanks for the help, Erick

Please include a URL to the page in question and FTP access if possible so I can see how you implemented the CAPTCHA.

Your validations weren't set up correctly, so the captcha would never fail. I fixed that.

I also added a simple question captcha with the image captcha. They are actually harder for bots to bypass than the image captcha.

I don't think spam will get through anymore, but let me know if you have any questions.

Thank you so much! Not sure what happened with the validations but I really appreciate your help fixing it and adding the secondary question!
