Update Behavior Not Updating

I've attached my form. I'm just testing a few of the form fields, lines 639-694.

The form is showing the values from the database table, but changes aren't being stored. A sample text label and input looks like this:

<label class="form-label mb-0">Name or Description of Site:</label>
<input name="si_site" type="text" class="form-control" value="<?php echo($rsAppl->getColumnVal("si_site")); ?>">

And my select list looks like this:

<label class="form-label mb-0">Site Class:</label>
<select name="si_class" class="form-control">
<option value="<?php echo($rsAppl->getColumnVal("si_class")); ?>" selected><?php echo($rsAppl->getColumnVal("si_class")); ?> </option>
<option name="default">select your site class</option>
<option value="Residential">Residential </option>
<option value="Commercial">Commercial </option>
<option value="Industrial">Industrial </option>
<option value="Construction">Construction </option>
<option id="admOption" value="Other" name="Other">Other </option>
</select>

On line 657 you have:
<form action="forms_application.php" method="post" enctype="multipart/form-data">

I think it will work if you update that to:
<form action="" method="post" enctype="multipart/form-data">

A blank form action will automatically use your current url including the url parameters, which would be necessary to make your update work properly. Currently your form submit isn't even going to the page where your update code resides.

I have another form using the Update Behavior. The queryobj.php debug is telling me I have an "Incorrect integer value." The only field with an integer in the table is the primary key. The update code looks just like the other Update Behavior code in my other forms.

The page is attached. Any help would be appreciated.

Update:
<form action="forms_final_inspection_edit.php"

to:
<form action=""

By setting the action it is removing the url parameter when the form submits which your update is trying to use to find the record to update and is the only integer referenced.
