upload image not working

I have a page with a form to update a table (persons); the update using the mySQI server behaviour works properly except for the file manupliation (uploading an image as a file). I have read through the webassist forum and tutorial but can't find the answer as to why the upload is not working. I have attached the .php page and also will sent you in a private post the link (I don't have security added to my pages yet that need to limit who can go to these pages).

File uploads require <form> tags to have: enctype="multipart/form-data"
