How to Upload file(s) in Javascript Ajax call?

I have to get the name of the file to be uploaded in the input filed on an onChange event (I think), then, on an onClick event that inserts a note (already working), I need to get the name of the file that was selected in the upload field, and Upload it after the Insert function. Attached is the Ajax file called (which works fine). So, I just need to get the 'attachment' and also have it upload on this function after the Insert. Let me know when you have time. Thank you.
