Dynamic select dropdown not populating

I'm having an issue with creating dynamic select dropdowns that do not list their respective table's content. Please see my webpage (attached). Could you review and see where I'm going wrong? This is low priority so there's no rush or urgency on this but I would like to know why it's not working. I have created two other dynamic selects that do work so I know the MySQL dB, and Databridge are OK.

It looks like you are populating your lists with the values from rsNewsletter... .but that recordset is being filtered to have only one month and one year. I think you would probably want to create a new recordset to get all months and all years in order to populate the lists.
