2 step Captcha problem

I have implemented Captcha into a website. This alone was not stopping bot submissions so i attempted to implement a second step verification (question). For some reason in Dreamweaver, everything looks fine but when I look at it live, everything below the 1st step catpcha is missing. I've attached the relevant files for review. Any help would be greatly appreciated.

Is it possible you didn't upload the file:
webassist/captcha/wavt_captchasecurityquestion.php

If you give me a url to look I might be able to debug it better.

I just checked and it looks like I don’t even have that file. Not sure how that happened? The only one I have is referencing wavt_captchasecurityimage.php

Here is a copy of the file. I'm not sure why it wasn't created, but you can just move it to the correct location manually.
