Surveyor does not Launch

When I specify that I want to create a new XML map an click on the finish button I get a JAVA error

"At line 160 of file "C:\Program Files\Adobe\Adobe Dreamweaver CS4\Configuration\Commands\WA_SurveyorXML.js":MM.WA_createProcessingObject is not a function.

After that happened I un-installed the extenion and Surveyor104.mxp. I did close and re-open DW CS4 and still get the error.

NOTE I did delete the winfilecache and that made NO Difference.

Found the issue

We generate our index.htm page so that did not exist on our local site. When I copied of version of our generated page Surveyor did load correctly.

The error messages should be "can not find xxxx.htm starting page"
