I'd need FTP access and a url to look at it.
The tokens in your email body file were wrong... They are supposed to be:
User name: [UserName] Password: [UserPassword]
I updated that and the email had them replaced.
However your forgot password server behavior has the username column set to MUSICIAN_ID instead of EMAIL, so it put the ID in the email and not the correct username. I updated it on the forgotpassword page.
The "Username" in this case IS the MUSICIAN_ID. To login the user has to enter their MUSICIAN_ID and password.
The form should return the MUSICIAN_ID and the password. This is why it was set the way it was. It works fine on my other sites, but here would not return any values in the return email.I have attached forgotpassword.php that is in the members/main directory of the same site. It works perfectly. I don't understand why the forgotpassword.php with the same relative settings does not work in the members_1 directory.

I updated the live page to use the MUSICIAN_ID in the email. I tested the page and the captcha seemed to work for me. When I entered bad captcha values the form would not send the email.

I am sending you the forgotpassword.php that i first used - that didn't work. I can't see any difference in what you did and the file that i used orignally.

I don't see the difference, but the one live was the one I had updated yesterday, so maybe it had an issue. Anyway, just download the one currently up there and you should be good to go.
I did download it I saw one difference orig did not have <?php require_once('../webassist/mysqli/queryobj.php'); ?> would that be the reason?
no that isn't it. Maybe I updated the include file to the latest version when I updated from my machine?
