I need help resolving a generic error message or registration page

I just ran the SA wizard and am testing my registration page and am getting a generic error.
Invalid information, please check your entries and try again.

My customers have complained about this error on my other sites as well and I have trouble duplicating it to resolve it. I am hoping you have a solution for the generic error message where it will tell me what content is invalid.

I have very few fields on my registration page, First Name, Last Name, Email, Username, Password

I am attaching my create account (registration page) and included a link to the live page as well as FTP info in the private message.

The latest versions of php seem to have an issue with the security question CAPTCHA. I had to re-organize the code on the page to fix it. I updated line 190 and added lines 30-33.

Do you have an example of the code you revised that can be used by others?

I need to update DataBridge to fix the issue. I had to update the include file and re-arrange the code on the page. The example code is in your file on the lines I mentioned.
