Unable to get the rename file name to be saved in the database table

Dear Sir. The extension seem to have an error, or I might me doing something wrong.
In the attached zip file, the file upload function uploads and renamed if the file exist in the defined folder, however I cannot get the new name to the file to saved in the database table. Instead the original file name is saved in the database. The error is only encountered when using Update record function

Insert Record to database table works fine.

I have the similar function using insert record function and it works find, I am able to upload and rename the file and Insert the new name in the database table.

Just when i am updating the record I am getting the problem

The file and the database sql file is attached and can be used to create the database

On line 26 try:
'FileName' => "[FileName]",

Instead of:
'FileName' => "".((isset($_FILES["FileName"]))? preg_replace('/.[^.]*$/', '', $_FILES["FileName"]["name"]) :"") ."",
