Web form

Hello Ray,
Point 1
I have a form at https://www.accessadviser.uk/mailform.php
When I fill the form in the last question labelled answer keeps asking for its answer.
I am attaching the code.

It's a bit strange because this worked previously.

Point 2
I am using Norton AV and when this form opened recently a Norton form appeared saying
Scam Insight: Personal Information Risk

Can you think of anyway to overcome this, seems to be a killer for the form.

I'll need FTP access to debug why the question field isn't working correctly.

To fix the norton issue, a google search said:
Please note that your site needs to be registered and site ownership verified to get the site rated. See Getting started with Norton Safe Web for more information.

If your site is currently untested by Norton Safe Web a gray icon appears on the site report page. To submit your site for rating:

Sign in to your Norton Safe Web account.
On the Site Dispute page, click "Rate my site" next to the site name. Note that the initial evaluation process takes about two weeks to complete.

Thanks Ray for the help with Norton.
I should have known this as I have carried out the process previously.

I will come back with the ftp a bit later as I am making some changes here.
