Not able to get Progress Bar to work

When the Progress Bar is added to my form, nothing happens when I try to submit. The Progress Bar just spins.

There could be another javascript error on the page causing it to fail. I'd need FTP access to debug since you have a few dozen script files at the bottom of the page.
