Why does my 'forgot password' show 'invalid information' error > URL: invalid=true

There are two areas to enter emails on my forgotpassword.php page - not sure why: Forgot Password and Please enter your email address to receive your password.

Entering an email address in either one shows the above error, though the email is in the database. I looked at the code to see if one could be deleted, but the code is not identical.

Attached are screen shots from the wizard, along with php page.

You had run the wizard multiple times on the same page. It was failing validation from a previous implementation. I cleaned up the page by deleting the repeated code and it appears to be working now.
