Updated DA form redirects to blank page after submit

Hi, I recently updated an existing DA form page to add a few fields. The form submits fine, stores all fields in the database and even sends the contents in an email. However, the user is not being redirected to the thank you page as specified in the $Email->Redirect value, but instead loads the same form page, which is now blank.

I think I remember experiencing this issue before, which may have been caused by an extra space at the end of a line of php code? But I've double-checked those and can't find any. Attached are both old and new files for comparison. Thanks!!

This code at the top would cause the issue:
<!--Revised 2020-02-14-->

comments in php should look like:

<?php // Revised 2020-02-14 ?>
