Need help with a Selected Item to MySQL table.

I'm trying to populate a MySQL database table via a PHP form. I want to insert a Response from an HTML editor field and a selected Title from a drop down field but I'm having a problem with properly setting up my code to accomplish this. The Response inserts into the table but the selected Title does not. How do I get the selected Title to be inserted into the table?

I am attaching the page for your review. Any suggestions as to what I'm doing wrong would be greatly appreciated.

It looks like your Title form field is in a different form. Only one form will be submitted at a time, so you would need all form fields in the same form that you want inserted at the same time.

YES!! Your Suggestion has Solved the Issue

Your eagle eye has picked out one of my coding booboo's. I combined both forms into one and now the form works perfectly. I've been looking at this for three days and did not catch that so a thousand thank-you's for your help. Again.

This issue has been resolved.
