form within repeat region submits forms

I am rebuilding a web program to update to php 7. The issue I am having involves a form within a repeat region that submits to all forms when it is submitted instead of just the one form that the data was input. It worked when I built it with the native server behaviors in dreamweaver, but does not work the same using the webassist mysqli server behaviors. Any assistance would be appreciated.

Probably the <form> tag should be outside of the repeat region. Only one form will submit at a time, so if you want all of the form elements to submit you need to have them inside a single form instead of multiple forms.
