Captcha implementing problems

I am trying to add captcha to a contact form that we've created. When I click into a cell in the form and go to insert captcha, it actually adds it but completely deletes the form. I have attached some screenshots so you can see what's happening. Side note, when I open the "insert captcha" window and try to change the font and background etc, nothing changes as I change the appearance. Last piece, we have tried using captcha many times on contact forms before this and have yet to have success in getting it to work. Maybe user error in using this tool?

It looks like you have the whole table cell highlighted when you apply. Try just putting your cursor inside the field. Maybe even type a couple of characters to make sure.
