A text field is missing

You helped me setup this landing page so when a visitor comes to this site they have to enter a password to get in. All the sudden the text field for the password is missing. Originally we had a password and username but I commented out the username text field as per their request. Could you take a look at the code and tell me where this has gone wrong. This is only happening on my computer, not on the posted web site. Hera are the files I believe you need. If I I missed something please let me know.

It looks like the field has been deleted from the page. You will need to add it back. Just add a field named "Password" and it should work.
