This form is working. However I need to edit it and it's telling me that there are improperly code values (see screen shoot below) and then when I look it's obvious where the problem is. So I edited and added them and got a blank page (I undid the error). However I need to get the page so that the redirect goes to somewhere else. (I would have just updated the redirect page but the problem is it needs to go to the same page with $_GET['ID'].) I have tired to manually add it and then Dreamweaver just starts to hang telling me about some script running in the background.

It looks like you wiped out all my changes. I redid them and edited the page. First thing I'm wondering why there is no $_GET option. Second editing and saving has caused the page to go blank now.

There was a syntax error in the redirect code on line 42. I've fixed that and the page loads now.
