Progress Bar Doesn't Show Up

I've been trying to use the Progress Bar but it's not showing up. What is weird is that if the form completes the upload and email submit and goes to the success page when I click the browsers back button the progress bar shows up. I've included the file. Thank you in advance.

I'd need FTP access and a URL to reproduce in order to debug this. It could be a javascript error of some sort, but I don't see an obvious problem in the files.

I just tested and I see the progress bar. Maybe it is a cache issue?
