Image insert not working in HTML editor

I am using the HTML editor in a form to send email blasts. I'm trying to embed a photo in the HTML content area, but when I click on the image icon from the HTML toolbar, then click browse, I get this error:

Not Found
The requested URL /index_cms.php was not found on this server.

Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.

There is no index_cms.php in my site. Thank you.

It is trying to make you log in. It looks like your files may have been copied from a site with powercms and it is trying to take you to the login page for the cms. If you open up the page:


You can update that page and change the login page specified and update the security rule to match your current site.
