Form as Include file doesn't execute

I have a form that inserts data into the database and sends an email. The form doesn't work when used as an include file. It works fine on it's own but not when embedded. <?php include("survey-questions.php"); ?> I have another include file form on the same page and it works fine.

You can see the form at http://scmserv.com the form is on the bottom right, it is a survey.

It should work. Have you tried using require() instead of include() ?

Yes and it still didn't work. I put it in an iframe and it works.

It probably had some sort of conflicting CSS on the page it was included on. I'd have to look to see, but an iframe solution is probably good enough. If you want me to look into it at some point I'd need a URL and FTP access to debug the broken page.
