Data Assist Updated page and upload file

I'm sure there is something that I am overlooking but I can't get the file to upload or the database updated. I've tried everything for someone that doesn't know php coding. Could you please take a look. Thanks

Your form tag is missing the enctype attribute. It needs to be set to enctype="multipart/form-data".

Add that to your form on line 330 and it should allow the uploads to process.

Its is working now but doesn't go to the detail page after the update.

Here is the error I'm receiving.
Warning: Cannot modify header information - headers already sent by (output started at C:\xampp\htdocs\ElvisRemake\elvisadmin\puppies_update.php:39) in C:\xampp\htdocs\ElvisRemake\elvisadmin\puppies_update.php on line 189

I found the problem some code wasn't in the proper order.

That is because of the blank line on line 38. If you delete that it should redirect correctly.
