Cannot get security module work

OSX 10.12.12, Dreamweaver CS6, Webassist

Please visit ipcoa.net. On the homepage click on the "admin_portal" link in the footer... this page and the resulting logon and logon error pages are not working. I cannot get the restrict page or logon page to work. I am including the pages (Ziped) for your review. Has this module been updated within the last year? (I have never been able to get Webassist's logon behavior to work properly...

I get a blank admin_portal page on the web site. Can I get ftp access to debug? I'll probably need that to get anywhere.
