Help with search

I have been looking at creating a search that uses a combination of standard search fields (in this case just one first of all), and also some checkboxes displaying values from an array.

I just about have it working - the checkbox part seems to be working OK now, but the single text field part doesn't seem be, which you'd think would be the easy bit.

I've been looking at it for quite a while now, so would really appreciate a second pair of eyes on it.

Just to let you know I had another logo at this after a break and spotted the issue - was the name of the button. Didn't catch it before as the checkboxes were working. Not sure why that was, but its fixed anyway.
