Fields missing A2C bindings tabs

I created a DW template with an Add to Cart button on it, and have created roughly 50 pages. On each of those pages, when I open the A2C GUI, there are 3 fields missing from the bindings tab. If I process a transaction, the data from those fields is recorded in the order details table, but they do not display in the cart where I have added them.

Any help is appreciated. Page is attached, live version is @

Fields missing are "School", StudentFirst, and StudentLast

It reads the available columns from the cart object. Download the file: WA_eCart/R4F14_PHP.php from the live site and it should correct this problem. My only guess is that you have a local version without those columns created.
