Change your form method to "post". Your email triggers on form post but the form is missing the post attribute: <form method="post">

how could I forget such a vital construct ---- will correct and see if that is the only issue. Thanks.

I have made suggested correction ---- form appeared to send, but not submission email is received.

What are you using as the FROM address? Can I see a copy of: webassist/email/waue_contact_1.php

It could be caught in SPAM filters as well. Make sure you are checking spam folders in the recipient email account.

ANS:

a) online-request@houseofannacouture.com
b) webassist/email/waue_contact_1.php is attached
c) emails were not found in spam/junk inbox

You need to set it to:
online-request@houseofannacouture.com

thanks --- you are so right. There is still one more problem. All expected contents didn't get sent, only the message portion got sent. please see below. I wonder why this occurred.
