how to hide a checkout form

Hello, can you please tell me how to hide checkout form that I add to the checkout form. Example: when I pay by credit cart I got a confirm cart and it hide the form pretty good but I Add 3 fields in the ecommerce peset by separate and the new fields stay out with Payment Options div. If you don't mind help me please.

I've updated the paymentoptions.js file to remove the Extras fieldset with the billing and shipping.
