Form not being emailed

My form looks like it's going through but is never received. Can you see what might be wrong?

I don't see anything inherently wrong with the page. Maybe try updating the FROM to use an email address that is registered with the site and uses the same @domain as the domain of the web server. Sometimes hosts prevent emails from going out that aren't from a locally registered valid email address. You can independently set the reply to address to the entered email from the contact form so that replying will still work.

It looks like it would work. Are you sure it isn't getting caught in a spam filter? Who is your hosting provider? Maybe you have to use the "extra headers" option or set the outgoing server with your hosting provider.

If you could give me FTP access I could do a bit more debugging. Please post FTP information in the private area and I'll do more testing.

I added debug code to the page and see this error:

Failed to connect to mailserver at "bosmail.nt.com" port 25, verify your "SMTP" and "smtp_port" setting in php.ini or use ini_set()

You will need to talk to your hosting provlder about setting the correct smtp server to work with php mail().
