Only Logout page show,rest pages are blank

Hello everyone,
I thought i be able to completed the membership login form.
Still working on it and got this problems.

Only Logout page will show on web and rest pages is blank on web.
I attached the code i used and show error message.
Please read my error message and give me advice...thank you for your time.

Also is there someone out there who live in Orange, Los Angeles County or live in San Diego who be able to show me to completed this project????

In your connection file, you entered an extra ' for the host name:
$hostname_conn = "'lasample.ipowermysql.com";


should be:
$hostname_conn = "lasample.ipowermysql.com";



The error you are getting on the code the host gave you indicates that the user "sung" has not been granted access privileges to the database on the MySQL server. check with them on that error, they should be able to assist you in granting access privileges to the Sung user for the database.

I tried two codes that one from Dw and other from ipower.com.
Which one i should use or it doesn't matter.
