Why is the form not sending when submit is pressed?

when the membership form is filled out and sent --- no email is received at the designated emails.

Q: Any idea why this is happening?

I don't see a problem with this page. Can I see a copy of the page: webassist/email/waue_membership-applicationwoo_1.php

Maybe it is where the issue is.

Are you on your live server when testing? What did you set as the FROM address? Are you sure it isn't sending but being placed in the SPAM folder?

One good way to test is to add a session dump to the page like:

php:
@session_start();

var_dump($_SESSION);



That will let you check to see if the page settings are set to the values you would expect and if it "thinks" the email was sent correctly.

You can add it to the body of the page that is displayed after the form submits.

there were no dump. only the thank you page loaded.

you are right it went into the junk box. but there is a problem --- none off the "filled in" data showed of the email/report. In the attached image you will see what came through.

Your form elements don't have name="" attributes. You need to add a name for the form elements for them to work properly.
