Can't log in using Security Assist - errors on page

I am attaching a zip file of login.php for you, as well as a screen print of the errors I receive when setting up my login pages. The login screen itself does not allow login. As soon as I tab out of the password field, I get a pop up that says "Please enter a value." It doesn't move to the next screen. Thank you.

I started with a new site, same database, but created ALL NEW login files. It's still not working. I keep getting error messages and invalid user name or password even though I am using the SAME database and the SAME credentials.

I'd have to have FTP access and a live URL to test this. Since it compares values in the database and the issue implies the record you are trying to find isn't there, it wouldn't be possible to debug without live debugging.

I just could not get it to work, even when I created a whole new site. I ended up just using the DW built in server behaviors for authentication. I just didn't have time to spend on getting it to work right now. Thank you.
