order details blank dialog screen

I am running Dreamweaver, CS4, Win 8, eCart 6.10.0 with a tested database connection. When I go through the checkout wizard and click on Define for the Order details table preset: I get an empty dialog box that I cannot do anything with. The very first time I ran the wizard I did not do anything here and it did create the default database structure for my connection. However I found I am not saving everything I need in the database thus I need to add to the Order details table the proper information. I have included a picture of my error. I have tried deleting the cache, no help. Un-installing and re-installing eCart, no help. Deleting all of eCart from my website and rebuilding my entire cart from scratch, no help. I am at a loss. Last step in completing a simple cart. I have tested this in the sandbox and now (after all my re-write) it processes the order but it does not save all the info in the orders table and of course I still can't access the order detail table to save all I want there. But at least is does open, increment, write, etc. as expected.

Please post a US phone number or Skype contact information if you are international. I'll have to debug this in a connect meeting because I can't reproduce it and don't know the cause.

We looked into this in a connect meeting. My only guess is that the large number of columns added to the cart was causing display issues. We worked around the problem by applying the checkout wizard with the defaults and editing the store cart details server behavior directly on the completed page.
