I'd probably handle this with some custom code on the cart_confirm.php page. Have it check the country selected and store the appropriate shipping charge as a session variable based on your rules. Then just set up a charge in the shopping cart as a flat rate charge equal to the session variable value with a condition of the session variable being set.
