Form will not submit to database since update

One of my forms stopped submitting to the database after updating to DataAssist 1.2.2
Now I'm unable to even create a new form from scratch that will submit data to the Database.

ok, looking at the code, one issue I see is this code at line 60:
<?php require_once("webassist/database_management/wa_appbuilder_php.php"); ?>

needs to be moved to line 1.

if the problem persists,
Please provide a Skype username or a US phone number and a good time to contact you when you will be in front of your computer and we will have an engineer contact you to resolve this issue over the phone.

We are available Monday - Friday from 9am - 4pm EST (except major U.S. holidays).

Ok, looks like I got it working by adding that line of code.
Couple of things though...

Why wouldn't DataAssist Form builder, build the form correctly in the first place since I was building it from scratch on a blank page? That means I'll have the same issues to any forms I build in the future and any current forms that I edit.
Also, when I clear the require check box for a field, any field, it doesn't stay cleared when I re open to edit the form.
Also, there's no way to change a check box form 0,1 to Y,N any more. Nor do I see a way to make the check box initially checked.

I spent all day working on this simple form...

Dreamweaver will occasionally place include files in the wrong spot. I'm not sure the cause but it is usually inconsistent. If it keeps happening then we can do a screen sharing session to investigate it.

The required field settings not sticking is a known issue that we will fix in an upcoming release.

You can set the initial state of the checkbox from the dreamweaver property inspector directly after creating the form. You can also update the checkbox to Y/N by updating the server behavior on the created page after running the wizard.
