WAFV_Required_C.htm error message

I'm using the new Client Side Validation from the CSS Form Builder. I'm adding the validation to a fresh form page.
I get the following error immediately when I attempt to add any Required options.

While executing onClick in WAFV_Required_C.htm, a Javascript error occurred.

I am sending a copy of the WAFV_Required_C.js file that should address the error.

Please copy it to the following location:


Win Vista:
C:Users\<Your User Name>\AppData\Roaming\Adobe\Dreamweaver 9\Configuration\Behaviors\Actions\WA Validation Toolkit\Apply Client Validations

WinXP:
C:\Documents and Settings\<Your User Name>\Application Data\Adobe\Dreamweaver 9\Configuration\Behaviors\Actions\WA Validation Toolkit\Apply Client Validations

Thank you for your response.
I did as instructed and this did not fix the problem.

I went ahead and killed the \Application Data\Adobe\Dreamweaver 9\Configuration directory and re-installed all of my extensions (1 hour later) I still still have the same problem.

What's happened to my Validation Tool Kit?

I still have the same issue after placing the file in the following directories.
