In Dreamweaver CS4, go to Commands -> Manage Extensions. Are SecurityAssist and Cookies Toolkit listed in the extesnion manager? Is there a check mark in the enabled column for both of them?
If so, they should be available under the Insert menu: Insert -> WebAssist - Security Assist and Insert -> WebAssist -> Cookies Toolkit.
If you continue to have problems, please post screen shots of the Extension manager and insert menu.
I have created a support ticket for you so we can look into the cause of this issue.

In trouble shooting this we noticed that different extesnion where enabled / disabled when accessing the extension manager from the Commands -> Manage extensions menu in Dreamweaver versus opening the extension manager from the start menu.
An extension that was enabled using the Commands -> Manage extensions menu in Dreamweaver would show in the toolbar, however an extesnion enabled using the start menu would not.
I recommended always accessing the extesnion manager using the commands menu, And contacting Adobe support to troubleshoot why there where multiple Configuration profiles.
