I have created an online survey form which includes an upload photo field. The results are emailed, how would I include a link to the uploaded photo in the email that is sent?

use the Server File Name binding in the email for the file name portion of the link:

<a href="http://yoursiteaddress.com/uploadfolder/<serverfile name binding>">link</a>

I've tried adding a hidden field to the form:
<input name="Photo_Front" type="hidden" id="Photo_Front" value="http://localhost:8888/champion_nutrition/client_photos/<?php echo $WA_DFP_UploadStatus["WA_UploadResult1"]["serverFileName"]; ?>">

And also adding it to the email body (webassist/email/tamplates/Block_2.php):
<tr valign="top">
<th style="font-family: Arial, Times, serif; font-size: 12px;width: 190px; text-align: right; padding: 3px 10px 3px 3px; color: #666; font-weight: bold;"> Photo (Front):</th>
<td style="font-family: Arial, Times, serif; font-size: 12px;padding: 3px; border-left: 1px solid #DDD; color: #666;">http://localhost:8888/champion_nutrition/client_photos/<?php echo $WA_DFP_UploadStatus["WA_UploadResult1"]["serverFileName"]; ?></td>

Neither work.

the hidden element method defiantly won't work.

in the template, try adding the following at line 1:

<?php global $WA_DFP_UploadStatus; ?>
