Form builder doesn't retain css

I've remade this form 3 times now. http://www.conejo365.com/photocontest.php

Each time, it looked great in the Form Builder window and in my Dreamweaver. I uploaded it and it looked fine except for those darn borders and bullets. I've done everything I can to remove them in the CSS Form Builder, but they don't go.

I remade the saved form elements into a new form and this is what happens. I've uploaded the entire webassist folder, but it doesn't retain my CSS styles.

I don't understand why it isn't remembering the way it looks.

Appreciate the help.

EDIT:
I've now uninstalled all WA extensions (Free Tools, Design Extender and Data Bridge) and reinstalled in the order that Jason has taught me: Tools, Design and Data Bridge.

This is now attempt #4.
I removed the form from the page and went into the Webassist New Form.

The form is a few simple fields. I recreated them.

I went into the CSS part and all I did was a few minor tweaks:
Changed form width to 100%
In Fieldset, I made the border Style None, width 0 pixels. I don't want the border or the bullets in the List.

In the Attachment 1, you will see what it looked like when I was finished. It looked like what I wanted. No Border and the fields all nice and neat.
Attachment 2, is how it looked after I saved the Design and returned to the main page.
Attachment 3 is how it looked after I re-uploaded the page and the entire Webassist folder.

It doesn't want to work for me.
Again, I appreciate the help.

actually, i think i see the problem,

The form name and ID will be generated using the preset name, your preset name starts with numbers:
365 Photo Contest

When naming a form or form elements, you need to conform to javascript variable naming conventions which do not allow for the starting character to be a number.

follow these naming conventions:
-Only use Letters and numbers. the only exception being the underscore
-first character must be a letter
