Datepicker not inserting into database field

I have a registration page created using SecurityAssist. All Form fields and hidden fields are being inserted to the database table fields.

The datepicker is used to capture the subscriber date of birth. This is a required field.

The date is not being inserted into the database.

Please look at my attached page and kindly pointed me in the right direction.

to store the date in the database, it needs to be converted to the
yyyy-mm-dd format.

add the following code at line 1 to do the formatting:

<?php
if(isset($_POST["Registration_group_19_Date_of_Birth"])) {
$_POST["Registration_group_19_Date_of_Birth"] = date("Y-m-d", strtotime($_POST["Registration_group_19_Date_of_Birth"]));
}
?>
