Email pw not working

I have an email pw page that used to work and now does not. I have not made any changes to it. In DW there is a red ! by the SA Email PW behavior that says prior version, so I don't know if that's the problem, but I don't think so.

I've attached it if it helps to look at it.

I get no errors when using it, the password just never arrives. I got a complaint from another user and tried it and it doesn't work for me either.

Any help would be appreciated. Thanks.

I changed the email port to 2525 and removed the reply to address, and am now receiving the password email.

Thanks so much Jason! I dowloaded a copy of the page to my local server. I'll change the email address back now so you don't get HOA email! :)
