Form not submitting

I have been having some serious issues trying using web assist forms. Crashing and now the form submit button won't take it to the thank you page. Nevermind trying to add the email. That is a big issue too with the crashing so at least want to get this part working. Attached are the files.

all of the paths in this form are for system references.

This will happen if the form is saved outside of a dreamweaver site.

When Adding form builder to a page, the page must be saved within the Dreamweaver site.

Create a new page within your dreamweaver site, and add the form to that page.

The fact that the page uses file system paths indicates the page is not saved to the DW site, this is probably the cause of the Form Builder Crashing.

I am not working outside of dreamweaver. I saved the file outside of dreamweaver for you to view. I will resend it. Ok I have attached them with out the saving to desktop. Please review!

submitting the first form (/health-form.php): went to thank you, a var dump of the session showed success on sending the email:

["waue_thankyou_1_Log"]=>
string(77) "Sending To: swsbioscience@gmail.com, webgirlfl@earthlink.net... Success


Second form looks to be a jquery conflict.

in the error console, I see the following errors:

jquery-ui.min.js:6TypeError: 'undefined' is not a function (evaluating 't.isPlainObject(o)')
ddsmoothmenu.js:54TypeError: 'undefined' is not an object (evaluating '$mainmenu.parent().get(0).className=setting.classname || "ddsmoothmenu"')
2jquery.h5validate.js:753TypeError: 'undefined' is not an object (evaluating 'settings[action]')


The curent version of Form builder uses JQuery validation, the jquery errors that are occurring before the validation look to be caused by the smooth scrolling script. those errors are causing the jquery validation to fail.

Try adding the form to a page that does not contain any previous jquery like the smooth scrolling script.
