Form not sending
In the Form Function Send Email Settings the attached form does not send to the assigned email address. The form closes and goes to the Thank You page but I do not receive the email. Can you take a look at the attached file.

I am not able to open the attachment you sent.
Who is your host?
Depending on your host, you may need to set certain options in Universal Email.
Some hosts will work if you leave the SMTP Server setting on the Email Server Tab blank, others will require that you enter the SMTP server address.
Some will work using the standard Mail protocol, other will require that you use the Pear Mail for SMTP, some using Pear mail will require authentication.
Some hosts may require that the from address being used be a valid email address for your domain. Some may require that the option to include the from Email address be passed as an extra parameter.
To be successful with Universal Email, you will need to know the requirements of your host for sending an email using a PHP script.
Also, double check your Spam folder, it is possible that the email is being trapped as spam.

the to address uses a comma separated list.
to set multiple to addresses, click the To button to enter the advanced to settings, and add each one separately.
In DW I clicked on the Contact: Basic Default form tab then on Properties clicked on Edit Form. In the Form Content tab I set Persistence to Session since it always defaults to None. Then in the Form Function tab I clicked on the To: field and typed in ,emailaddress@url.com. This did not send.
Next, in the same file with two addresses I deleted the second address using Code View. (I always hate to do this in DW since I have to detach the template from the file.) The single address does not send either. Latest file attached

Looks like you are using the email address from the form for the from address.
many hosts require that the from address be a valid email address for your domain.
To edit the universal Email settings, open the server behaviors panel and double click the Universal Email server behavior.
For the from Address, Click the From button and set the from address to use a valid email address for your domain.
for the reply to address, you an click the lightning bolt icon and select the email address form element.
you are also leaving the SMTP server and port settings blank, you should double check with the host to see if this is valid. by leaving it blank, it will use the SMTP Server and port from the php.ini file. if these are not set, leaving them blank wont work.

I rebuilt the attached form file and it is still not sending. Deleted all files and folders on local and remote before starting. One thing that is happening that I might not have caught yesterday is you can enter anything into Security Code or Answer fields and not get an error notice. If you enter correct answers the file closes and goes to Thank You page. But does not send.
Did add SMTP address and port.
