Forgot Password page not showing error messages correctly

Hi!
As I was testing through the SA generated pages, I noticed that when I entered an non-existent email account, I was taken to a 404 Error File Not Found page when it should be showing the No Record Found message.

I have attaching the file.

Any help is greatly appreciated!

i updated the success and failure addresses in the forgot password file:

"successRedirect" => "/accountaccess/login.php?emailedPassword=1",
"failRedirect" => "/accountaccess/forgotpassword.php?notFound=1",

OK, didn't realize this was happening for email addresses that did not exist.

I updated the webassist/security_assist/helper_php.php file to fix the error.
