Unable to receive file been uploaded. not getting sent to mail.

i got a form, where in a user upload his resume online. so that form i want it to be sent to mail. everything is getting sent but FILE uploaded i m not receiving.. i m just receiving file name. thats all. ex: in mail file: exmple.jpg
I crossed checked many times. don't know where i have gone wrong. please help me soon .
In attachment i choosed : SUBMITTED FILE as well. still. file is attached. please take a look.

Hey it got solved. Now i m receiving files as well.i guess it had happened cos of this :enctype="multipart/form-data" in form.
Superb cool now.

By the way i have created another file field which is hidden and through that i want to know the exact link where user drops the form from.. can you tell me what server variable needs to be used to get exact link where user had dropped FORM.
exmaple. www.domain.com/exm.html
